Medications

Should it be necessary for students to receive medication prescribed by a physician during school hours, we are prepared to assist.  Parents are required to have the physician complete a “Request for Administration of Medication by School Personnel” form.  These forms are available at the school office.  Parents are also responsible for completing an authorization form and for instructing school staff in the administration of the medication, its proper storage and an outline of any emergency plans which may be required.

The medication will be delivered, according to an agreed schedule, to the principal or designated person for safe-keeping.
